As the parent, legal guardian, or agent of the above-named participant (the "Participant"), and for myself, my heirs, personal representatives and assigns:
1. I represent that I and the Participant fully understand the nature of the activities in which Participant will be involved during the Denver Broncos/Gatorade Jr. Training Camp (the "Camp") which may include, without limitation, athletic lessons, fitness testing, warm-up exercising, practicing football skills, running football drills and transportation to and from the Camp (the "Activities").
2. I represent that the Participant is presently in good health and in suitable mental, physical and emotional condition to fully participate in any of the Activities. In the event of an emergency, the Participant can be treated either by the Denver Broncos Football Club or at a local hospital.
3. I fully understand that the Activities involve substantial and serious risks and danger of bodily injury, including, without limitation, permanent disability, paralysis, and death, and of property damage. Such risks and dangers may be caused by, among other factors, action or inaction of the Participant, action or condition of the Participant, the condition in which an Activity takes place, the condition of the premises on which an Activity occurs, the condition of any equipment used in an Activity, or the NEGLIGENCE of the Releases named below. I and the Participant fully accept and assume all risks and responsibility for losses, costs and damages incurred in participation in any of the Activities.
4. I completely and unconditionally RELEASE, FOREVER DISCHARGE, CONVENANT NOT TO SUE and AGREE TO INDEMNIFY AND HOLD HARMLESS PDB, Sports, Ltd. d/b/a the Denver Broncos Football Club, Ltd, Gatorade and their respective officers, directors, employees, volunteers, agents, and corporate sponsors (each considered one of the "Releasees") from all liabilities, claims, demands, losses, and damages (including attorney's fees) arising, directly or indirectly, out of the Camp or any of the Activities, whether or not caused or alleged to be caused in whole or in part by the NEGLIGENCE of any of the Releasees, or otherwise, and whether such losses or damages can be anticipated or not. Further, my agreement to indemnify any of the Releasees shall apply, without limitation, from and after the date on which any minor participant attains the age of majority under Colorado law now or hereafter amended. As a participant, I consent to the use of my name, voices, pictures or other likeness for promotional purposes determined by Gatorade or the Denver Broncos, without further compensation, unless prohibited by law.
I have read this complete Waiver and Release, fully understand all of its terms, and have voluntarily signed it.
